There is no magic in Natural Penis Enlargement, only the effects of an action causing a reaction from the body. Cause and effect. But it’s not as simple as that. Performing this exercise requires a good sense of judgement as to how to apply a force onto your tissue. You are not exercising a rubber balloon, you are exercising a complex structure that is under the control of a complex series of reflexes, neuronal and humoral control. Our penis is formed by muscles cells (including the 'corpora cells' within the penis), which are sorrounded by thin sheaths of connective tissue. These layers can be pictured as exceptionally tough layers of plastic wrap snugly surrounding each cell (and groups of cells). In order for each cell to enlarge, the tough, fibrous connective tissue sorrounding the cell must be stretched to a larger diameter. Think of these layers as 'girdles' that restrict expansion. Those ligaments and cellular tissue stretch and heal to adapt to the new stressors on them. NO activates the enzyme guanylate cyclase that converts guanosine triphosphate (GTP) to cyclic guanosine monophosphate (cGMP). cGMP causes smooth muscle relaxation (probably by decreasing cellular calcium which is needed for contraction) with a resulting increase in blood flow into the sinusoidal spaces. cGMP is inactivated naturally by an enzyme, phosphodiesterase type 5 (PDE-5), which is present in the smooth muscle cells. Sildenafil (Viagra TM) inhibits PDE-5 and thus prolongs the effect of cGMP in cases where its natural production is insufficient to cause or maintain an erection.
NO is released from the both the non-adrenergic non-cholinergic nerves (NANC) that supply the penis and also from the endothelial cells that line the sinusoids. The endothelial cells are triggered to release NO by acetylcholine from parasympathetic nerve fibers that are active during sexual stimulation. This explains why conditions that damage endothelial cells, such as high cholesterol, diabetes, or smoking, may also compromise erection. The mechanisms that mediate NO release and cGMP production and action are thus key features in the process of erection.
Although it is thought to play a lesser role than NO in facilitating erection, prostaglandin E1 (PGE-1) is also released from penile nerve endings during sexual arousal. Like NO, it activates production of a compound in smooth muscle cells that causes muscle relaxation. In this case, the compound is cyclic adenosine monophosphate (cAMP). PGE-1 activates the enzyme (adenyl cyclase) converts adenosine triphosphate (ATP) to cAMP). These observations explain why PGE-1 injected intercavernosally (Caverject TM) or inserted as a pellet intraurethrally (MUSE) can induce erection in men with ED. Other naturally occurring neuropeptides such as vasoactive intestinal peptide (VIP) may also be involved in the smooth muscle relaxation that leads to erection.

According to his article, the importance of smooth muscles for an erection has been the subject of detailed analysis by physicians and doctors (e.g. The Journal of Urology and in article written by Dr. George J. Christ, which was published in The Urologic Clinics of North America).

And more recently, we are realizing that cyclic training using deconditioning breaks helps us keep the penis in a responsive state. And cyclic training is a popular weightlifting principle (if not the biggest).
Indeed, the penis is an extraordinary organ that has captured our attention since we were little boys. And PE is an extraordinary process. No one is for certain how penis enlargement works, but it is probably much more simple than we think.
By taking conventional wisdom (we know that muscle grows due to exercise) and comparing it to scientific facts (the penis is 50 percent smooth muscle and smooth muscle also grows due to stress) we can theorize that the smooth muscle of the penis plays some role in penis enlargement - I'm not sure how much, or how little, but at least some role.”
